We specialize in Oral and Maxillofacial Surgery, with specific expertise in dental implants, third molar removal, oral pathology, and various anesthetic techniques. Scheduling

Due to the variety of surgical services we provide, certain circumstances may lengthen the time allocated for any given appointment. Emergency cases have also been known to cause delays. We appreciate your understanding and patience.

First Visit

Your first visit with our Practice may consist of a consultation to determine your diagnosis and any/all applicable treatment options. Surgery can sometimes be performed without the need for a consultation, but a complicated medical history and/or treatment plan is more likely to require an evaluation prior to surgery.

All patients under the age of 18 must be accompanied by a parent or court-appointed legal guardian. Step-parents and grandparents are not considered legal guardians, unless specifically appointed by law. Substantiating documentation is required.

Please assist us by providing the following information prior to your appointment:

  • Your surgical referral slip and any applicable x-rays
  • Completed patient registration form
  • If you have medical and/or dental insurance, please bring your insurance cards and any additional completed forms if applicable

Pre-operative consultations and physical examinations are mandatory for patients undergoing IV anesthesia in our Practice. Please refrain from eating or drinking anything eight hours prior to your scheduled surgical procedure. You will need an adult to drive you home and stay with you for the following 24 hours. Specific instructions will be given at your consultation and surgical appointments.

Please let our staff know if you have a medical condition that may be of concern prior to surgery (including but not limited to diabetes, high blood pressure, artificial joints or heart valves, radiation therapy to the head/neck area), or if you are taking any heart medications, bisphosphonates, RANKL inhibitors, anticoagulants, or antiplatelet medications.

Patient Registration

Preregistration is available through our secure online patient registration form. Once completed, be sure to click the Complete and Send button at the bottom to confirm that your information has been provided to us. The security of your personal data is one of our primary concerns, and we will take every precaution to ensure it’s protected.

Insurance Information

At Armanini, Kolodychak, & Basile, L.L.P., we offer our patients superior surgical care paired with the most convenient financial options available. In order to do this, we work with our patients directly to maximize their insurance reimbursement for covered procedures. Please note that some procedures may not be covered under your medical and/or dental insurance plans.

Financial Policy

For your convenience, we accept cash, bank-certified/cashier’s check, American Express, Visa, MasterCard, Discover, and Care Credit.

Personal checks are not accepted.

Please remember that insurance is a method of reimbursing the patient for fees incurred at Armanini, Kolodychak, & Basile, L.L.P. and is not a substitute for payment. It is wholly your responsibility to pay any deductible, coinsurance, copay, or other balance not paid for by your insurance company.

Most insurance companies will respond within one to two months. Please call us if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. We will make every attempt to provide you with a reasonable estimate of the charges for any procedure beforehand.

In an effort to provide exceptional surgical care while keeping costs reasonable, payment is due prior to services being rendered. If you have questions regarding your account, please contact our office at (814) 838-2144.

Appointment Policy

No Show & Cancellation Policies for Office Visits & Surgery

Drs. Armanini, Kolodychak, and Basile, are committed to providing exceptional care to all of our patients. We understand that emergencies happen, and that there may be times when you must miss an appointment due to work or family obligations for work. Unfortunately, by not calling to cancel your appointment, you are preventing another patient from receiving much-needed treatment.

Please contact us at (814) 838-2144 at least 48 hours prior to your scheduled appointment to notify us of any changes or cancellations. In order to cancel a Monday appointment, please contact our office by 11:00 AM on Friday.

If your appointment is not canceled or rescheduled within this timeframe, you may be charged a $75 fee. This fee will not be covered by your insurance and must be paid prior to reappointing. Repeated no-shows, cancellation, or rescheduling, may result in dismissal from our Practice.

Account Balances

Patients with outstanding account balances are required to pay their balances in full prior to receiving additional services from our Practice.
